Provider Search Instructions
The Provider Lookup database contains all providers with an open DC Medicaid provider number. The information regarding address and telephone for each provider is what is currently on file with the Department of Health Care Finance and may not always reflect current changes.
To search for a Medicaid provider:
- Enter any combination of the provider's type, specialty, last name, first name, city, state, zip code or county. In order to select more than one type, specialty or county single click the first then select more by holding the CTRL key and clicking more. Select a range by holding down the SHIFT key when above or below items are all ready selected.
- Press the Submit button.
- If you make a mistake, press the Reset button and start again.
